STUD DOG ADVERTISEMENT: Stud Dog Register Form (PDF), Stud Dog Register Form (Word) - please include all required certificates and a photo of the stud dog to be listed.

BREEDER ADVERTISEMENT: Breeder Recognition Form (PDF), Breeder Recognition Form (Word) Supporting Documents for the Paw Print Scheme - breeder and/or stud dog owner must be a member of the Club for at least two years, have attended the breeder forum and/or reviewed the online content). Required to be submitted by the breeder and/or stud dog owner who is placing an advertisement through the Club. Breeder Recognition Scheme ('Paw Print Scheme') explained.

TITLE RECOGNITION: Title Recognition Application Form (PDF), Title Recognition Application Form (Word) to be completed for breeders/owners who would like to have their dog's title acknowledged in the Annual General Meeting. Each year at the Annual General Meeting, title awards are presented to members in recognition of the titles that they have earned with their dogs during the previous year (1 Jan to 31 Dec). Due date of submission is by 7 January. A copy of your dog’s title paperwork is required.

RESULTS SUBMISSIONS: Show & Performance Results Form (PDF) or Show and/or Performance Results Form (Word) complete results should be submitted to the Club by the end of the month.

COMMITTEE NOMINATION: Nomination Form (Word) or Nomination Form (PDF) Due date of submission is on or before 8 weeks prior to the scheduled AGM.

MEMBERSHIP FORM: Membership (new members or renewal) Complete this form and send it to either the Club Treasurer or the Membership Officer if you prefer paper-based application.
